Study: Inadequate information about the quality of nursing homes

    According to the Bertelsmann Foundation, important information about nursing homes can only be viewed by consumers in six federal states. Anyone looking for a facility is often at a loss – key facts are missing.

    Gütersloh – The majority of the federal states are providing consumers with an investigation according to no information about the quality of nursing homes ready.

    Important information such as the deployment of staff is available in all countries, but remained under lock and key in most federal states, the Bertelsmann Foundation criticized when the “White List” was presented. Many people who are looking for a nursing home are thus deprived of information on essential selection criteria. “The nursing home, as the most dangerous place for very old people in the Corona crisis, remains a black box,” said the board member of the German Foundation for Patient Protection, Eugen Brysch, the German Press Agency.

    According to the study, in ten countries data resulting from examinations by the regulatory authorities responsible under national law are not published at all. As a result, consumers do not find out whether there is a lack of staff in a home or whether there are serious defects, as the foundation complained about. Likewise, it would not be possible to understand which facilities are particularly well positioned.

    Only North Rhine-Westphalia, Hamburg, Mecklenburg-Western Pomerania and Berlin are actively publishing the results so that they are generally available. In Baden-Württemberg and Hesse, the test results would have to be published at least by the nursing homes themselves. However, the data can currently only be viewed directly in the facilities – i.e. only accessible to a limited extent.

    The lack of transparency arises either, because there is no state law with an obligation to publish or because existing laws are not implemented. In Lower Saxony, Rhineland-Palatinate, Saarland, Saxony and Thuringia, for example, publication of the test results is not required by law. And the states of Bavaria, Brandenburg, Bremen, Saxony-Anhalt and Schleswig-Holstein would not implement existing laws for various reasons, which was “particularly serious”.

    Hamburg stands out positively, where detailed information about temporary freezes on admissions or results from surveys of relatives are posted online. According to the latest information from the Federal Statistical Office, the number of people in need of care – as of 2019 – has continued to rise to more than 4.1 million people, about a fifth of whom live in one of the approximately 15,400 nursing homes.

    Choosing a home is a life decision, those affected need reliable information, emphasized Brigitte Mohn, board member of the foundation. It could also be used to make the good work of many nursing professionals visible to the public. “Conversely, it should also be possible and allowed to recognize the nursing homes where there are deficits.”

    Patient advocate Brysch complained that hygiene controls by the health authorities had been shut down due to the pandemic. Visits to relatives have only been controlled and made possible “in homeopathic doses” for almost two years. Federal Health Minister Karl Lauterbach (SPD) must now explain “when the information gaps in inpatient care for the elderly will be closed”.

    The GKV -Spitzenverband told the dpa: “A Heimplatz is not just any place, but the new home of the people in need of care who move in there. That’s why we don’t expect things to be done by halves when we provide information.” Vice-Chairman Gernot Kiefer appealed to all federal states to “ensure openness and transparency here”. The long-term care insurance funds have been publishing the results of quality tests for ten years, according to the National Association of Statutory Health Insurance Funds, which represents the interests of the statutory health and long-term care insurance funds.

    Foundation expert Johannes Strotbek explained that those in need of care have “in principle the right to freely choose the professionally suitable, individually suitable and qualitatively good service provider”. This requires an overview of the range of services and quality of the providers. At the federal level, core results from quality tests have been available for several years via the “Pflege-TüV” – with gaps.

    Open At the state level, there is a patchwork quilt with large information deficits. All federal states should also disclose the relevant data available to the supervisory authorities, Strotbek warned in the analysis. This could also provide impetus for quality competition among the institutions. Consumers, information portals, advice centers and healthcare research should be able to freely access information on the quality of care.
